Obtaining the admission form

1. To be obtained in person.
2. To be obtained by post.
3. To be downloaded from Sik Sik Yuen website.
4. Please click here and download.

Please provide these documents when submitting the admission form.

1. Copy of child’s Birth Certificate (hard copy for checking)
2. Copy of Immunization Record (hard copy for checking)
3. One Child’s photos (excluding the one on application form), write name at the back. size : 1.5 inch X 2 inch
4. Four envelopes with child’s name, address and $2.2 stamp
5. Application fees $40
